Who may be able to join

Who might be able to join this trial:

  • You are a cancer survivor who has finished treatment aimed at curing your cancer within the last 5 years (you may still be taking long-term maintenance medications, such as tamoxifen or aromatase inhibitors for breast cancer, or hormone therapy for prostate cancer)
  • You were diagnosed as an adult (age 18 or older) with lymphoma, breast, colorectal, lung, or prostate cancer
  • You are able to complete all parts of the study, including answering questionnaires
  • You are willing to be randomly assigned to a study group
  • You are able to give your informed consent to participate
  • You agree to allow the study team to access your cancer and primary care medical records
  • You speak English or Spanish

Who may not be able to join:

  • You are currently receiving palliative or hospice care, or you are planning to move to that type of care within the next 3 months
  • You do not have access to a telephone
  • You do not have a mailing address or a way to receive study materials electronically
  • You currently receive your primary care or cancer care in a pediatric (children's) medical setting
  • You have been diagnosed and treated for more than one type of cancer — though skin cancers removed by surgery alone do not count against this, and having a relapse of your original cancer does not make you ineligible as long as you have completed curative treatment and meet all other criteria

Initial participation rate of cancer survivors identified from community-based or partner practices, and Cancer Surveillance System of Western Washington; Accuracy of survivors' self-generated survivorship care plans (SCPs) in relation to those based on medical record abstraction; Proportion of survivors who receive the phone-based education session within the study time period; Proportion of survivors who complete the follow-up questionnaire within the study time period; Response rate among primary care providers (PCPs) to the PCP survey
