Who may be able to join
Who might be able to join this trial:
- You are willing to take part voluntarily and can sign a consent form, attend follow-up visits, and follow the study plan.
- You are 18 years of age or older.
- Your cancer started in the colon or rectum.
- Your cancer has been confirmed by a tissue sample (biopsy) to be a specific type called adenocarcinoma, and testing has shown it is of a particular genetic type (called pMMR or MSS — confirm with trial site if unsure what this means for you).
- Scans taken within the last 3 weeks show your cancer has spread (metastasised), with at least one measurable affected area in the liver, lungs, or abdomen, and a medical team has decided the spread cannot currently be removed by surgery.
- You are generally well enough to carry out light activity or normal daily tasks (a medical team will assess this using a standard scale).
- Your doctors expect you to live for at least 3 more months.
- Your blood, kidney, and liver test results are within acceptable ranges showing your organs are working well enough for treatment.
- You have no medical reasons that would prevent you from having surgery.
Who may not be able to join:
- You have previously received certain types of immunotherapy drugs (such as PD-1 or PD-L1 inhibitors or similar treatments).
- You have had any previous treatment for your colorectal cancer, including chemotherapy, radiotherapy, or surgery to remove the tumour.
- Your cancer testing shows a specific genetic marker called MSI-H or dMMR (a different genetic type to what is required for this trial).
- Your cancer has spread to the brain.
- You have taken part in another cancer drug trial within the last 4 weeks.
- You have had a previous allergic reaction to any of the drugs used in this trial (including tislelizumab, capecitabine, oxaliplatin, or bevacizumab), or to similar antibody-based medicines.
- You have had a local treatment to your liver or lung metastases (such as ablation, targeted radiation, or surgical removal of spread) within the last 4 weeks.
- You have a confirmed immune system disorder or an active condition where your immune system attacks your own body (such as lupus, rheumatoid arthritis, inflammatory bowel disease, or similar), or you have taken immune-suppressing medicines or steroids in the last 4 weeks.
- You have a known HIV infection, or a significant liver disease including active hepatitis B or hepatitis C infection at detectable levels.
- You have high blood pressure that cannot be controlled, even with three or more blood pressure medications.
- You have a condition that causes serious bleeding risk, a tendency to bleed easily, or a clotting condition that requires you to take blood-thinning or anti-clotting tablets long-term.
- You have significant heart problems, including moderate-to-severe heart failure, unstable chest pain (angina), a heart attack within the past year, or serious uncontrolled heart rhythm problems.
- You have had a serious infection in the last 4 weeks, currently show signs of infection, have needed antibiotic tablets or injections in the last 14 days (other than for prevention), have active tuberculosis, or have had tuberculosis in the past year (or longer ago if it was not fully treated).
- You have been diagnosed with another cancer within the last 5 years, unless it was a type with a very low risk of spreading or causing death (such as certain treated skin cancers or early-stage cervical changes — confirm with trial site).
- You are currently pregnant or breastfeeding.
- You have a psychiatric condition, significant cognitive (thinking or memory) difficulties, or are critically unwell.
- You have any other serious condition, or personal or social circumstances, that the trial doctor feels could affect your safety or ability to complete the study (confirm with trial site).
