Severe Crohn disease
Initial treatment
Patient must have received treatment with the IV formulation of this drug for this condition; or
Patient must have received an approved PBS authority prescription for treatment with the IV formulation of this drug for this condition; or
Patient must not have received treatment with the IV formulation of this drug, but has met the restrictions criteria as specified in the Initial 1, 2, or 3 for the IV formulation for this condition; AND
Must be treated by a gastroenterologist; or
Must be treated by a consultant physician [internal medicine specialising in gastroenterology]; or
Must be treated by a consultant physician [general medicine specialising in gastroenterology]; or
Must be treated by a paediatrician; or
Must be treated by a specialist paediatric gastroenterologist.
The following information must be provided for patients who have not received the IV formulation as loading dose(s), but has met the restrictions criteria as specified in the Initial 1, 2, or 3 for the IV formulation:
(i) reason for loading doses being omitted; and
(ii) if applicable, details of prior systemic therapy [dosage, date of commencement and duration of therapy]; and
(iii) if applicable, details (date, unique identifying number/code or provider number) of the pathology or diagnostic imaging test(s) nominated as the response criteria; and
(iv) if applicable, details of intestinal inflammation marker/s; and
(v) if applicable, the PCDAI or CDAI score and the date of assessment; and
(vi) the date of the most recent clinical assessment.
