Carcinoma of the prostate
The condition must be locally advanced (stage C); or
The condition must be metastatic (stage D); AND
Must be treated by a medical practitioner; or
Must be treated by a nurse practitioner where both of the following are occurring: (i) patient care is being shared with a medical practitioner, (ii) the prescription continues existing therapy with this medicine.

Endometriosis
The condition must be visually proven; AND
The treatment must be for the short-term (up to 6 months); AND
Must be treated by a medical practitioner; or
Must be treated by a nurse practitioner where both of the following are occurring: (i) patient care is being shared with a medical practitioner, (ii) the prescription continues existing therapy with this medicine.

Breast cancer
The condition must be hormone receptor positive; AND
Must be treated by a medical practitioner; or
Must be treated by a nurse practitioner where both of the following are occurring: (i) patient care is being shared with a medical practitioner, (ii) the prescription continues existing therapy with this medicine.

Anticipated premature ovarian failure
Patient must be receiving treatment with an alkylating agent for a malignancy or an autoimmune disorder that has a high risk of causing premature ovarian failure; AND
Patient must not receive more than 6 months' of treatment for this condition in a lifetime; AND
Patient must be pre-menopausal; AND
Must be treated by a medical practitioner; or
Must be treated by a nurse practitioner where both of the following are occurring: (i) patient care is being shared with a medical practitioner, (ii) the prescription continues existing therapy with this medicine.
