Who might be able to join this trial:

• People aged between 18 and 70 years old.
• People of any gender.
• People who meet the NHS criteria for weight-loss (bariatric) surgery.
• People who have been diagnosed with either prediabetes (impaired glucose regulation) or type 2 diabetes, based on World Health Organization guidelines from 2011.
• People with diabetes whose condition is stable and managed through diet alone or with a single diabetes tablet (not insulin).
• People whose HbA1c blood sugar level (a measure of average blood sugar over several months) is at or below 9.0% (or 74.9 mmol/mol).

Who may not be able to join:

• People with a medical, psychological, or other condition — or who take any medications, including over-the-counter products — that the trial investigators believe could interfere with the study or cause harm.
• People who do not have access to a telephone at home, or who have other circumstances likely to make reliable participation difficult.
• People who are pregnant or breastfeeding.
• People who smoke.
• People who are unable to use reliable contraception throughout the study and for one month after it ends.
• People who have had an allergic reaction to any ingredients used in the under-the-skin (subcutaneous) infusions involved in the study.
• People who have donated blood in the three months before joining, or who plan to donate blood before the study ends.
• People who currently use insulin to manage their diabetes.
• People with high blood pressure that is not well controlled.
• People with other serious health conditions that could affect the safety of the study or its results, such as heart failure or known cardiovascular (heart and blood vessel) disease.
• People taking blood-thinning medications such as warfarin or factor Xa inhibitors will not be able to take part in the fat tissue biopsy (a minor procedure to collect a small sample of fatty tissue).
• People who are unable to give informed consent to participate.
• People who have previously had bariatric (weight-loss) surgery.
• People who are unable to undergo an fMRI brain scan, including those who:
  • Experience claustrophobia (fear of enclosed spaces).
  • Have a pacemaker, metal implants, clips, implanted devices, shrapnel, bullets, or metal fragments in the eyes that are not compatible with magnetic resonance imaging.
  • Have a significant abnormality found on a brain MRI scan.
