- You were diagnosed with one of the following conditions at one of the participating hospitals on or after January 1, 2020: acute myeloid leukemia, multiple myeloma, hemophilia, aplastic anemia, leukemia, myelodysplastic syndrome, lymphoma, a bleeding disorder, or you received a bone marrow transplant.
