Who might be able to join this trial:
- People aged 65 or older who receive care from a local community health service centre.
- People who have been diagnosed with type 2 diabetes, confirmed by at least one of the following: typical diabetes symptoms with a high random blood sugar reading; a fasting blood sugar level of 7.0 mmol/L or higher (after no food or drink for 8 hours); a blood sugar level of 11.1 mmol/L or higher two hours after a glucose drink test; or current or past use of diabetes medication. (Each blood sugar result must be confirmed with a repeat test.)
- People who also have chronic kidney disease and/or a high or very high risk of heart or blood vessel disease, meeting at least one of the following conditions: a history of heart disease, stroke, or poor circulation in the legs or feet; signs of organ damage such as protein in the urine, reduced kidney function (but not below a certain level), an enlarged heart muscle, or eye disease related to diabetes; three or more major risk factors including age 65 or older, high blood pressure, abnormal cholesterol levels, smoking, or obesity; or a diabetes duration of 10 or more years plus at least one heart disease risk factor such as older age, obesity, smoking, an inactive lifestyle, a family history of heart disease, high blood pressure, or abnormal cholesterol levels.
Who may not be able to join:
- Women who are pregnant or planning to become pregnant.
- People whose kidney function is below a certain level, specifically an eGFR lower than 30 mL/min/1.73m² as measured by the CKD-EPI formula (confirm with trial site).
- People who are unlikely to be able to attend follow-up appointments for the full 36 months of the trial, due to health reasons or plans to move away.
- People who are unwilling or unable to sign a consent form agreeing to take part.
- People who have type 1 diabetes.
