Who might be able to join this trial:
For COVID-19 survivors:
- People who identify as African American
- People of any gender (male or female)
- People living in a medically underserved area or a designated rural area of South Carolina
- People aged 18 years or older
- People who were hospitalised, visited an emergency room, or visited an urgent care clinic due to COVID-19 at any point since March 11th, 2020
- People who have previously been diagnosed with one or more of the following conditions: type 2 diabetes, high blood pressure, heart disease, chronic kidney disease, or stroke (diagnosed more than 3 months ago)
For care partners (people who support a survivor):
- People of any gender (male or female)
- People aged 18 years or older
- People who live on the same property or in the same community as the survivor, preferably within 40 miles of them
- People who are the main unpaid provider of care or social support for the survivor at home
Who may not be able to join:
- Survivors or care partners who are currently enrolled in a related clinical trial
