Who might be able to join this trial:
- People who identify as Hispanic or Latino, of any race
- People who are registered patients at a federally qualified health center
- People who are 18 years of age or older
- People who have Type 2 diabetes with a blood sugar control measurement (HbA1c) above 7.5%, recorded in the last 90 days
- People whose primary care doctor has given approval for them to participate
- People who are not currently taking part in any other diabetes management or education program
Who may not be able to join:
- People who are currently receiving treatment for cancer or Parkinson's Disease
- People who are currently pregnant or breastfeeding (this is a temporary exclusion — those who are 6 months or more past giving birth may be considered eligible)
- People who have plans to move away from the local area within the next 12 months
- People who have mental or physical health conditions severe enough to prevent them from fully understanding and participating in the study
- People who have serious diabetes-related complications, such as kidney disease or those currently on dialysis
