Who might be able to join this trial:
For healthcare providers:
- Full-time primary care doctors or nurse practitioners who work at one of the participating health centres or group practices
- Providers who care for at least five patients diagnosed with type 2 diabetes
For patients:
- People who have been diagnosed with type 2 diabetes for at least 6 months
- People whose type 2 diabetes is not well controlled, meaning a specific blood sugar measurement (HbA1c) was above 7% on at least two medical visits in the past year

- People who are pregnant or planning to become pregnant within the next 12 months
- People who are currently taking part in another type 2 diabetes study
- People who plan to stop attending the clinic within the next 12 months
