Who might be able to join this trial:
- You are a patient at Penn State Health St. Joe's Medical Center
- You have been diagnosed with Type 2 Diabetes for at least 6 months
- Your most recent A1c blood sugar level is 7% or higher (a measure of average blood sugar over time)

- You have attended any diabetes self-management education or support sessions in the past year
- You are pregnant or plan to become pregnant during the study
- You have an untreated or unstable mental health condition or substance use problem, or have received inpatient treatment for these in the past 6 months
- You have had a serious or unstable health event in the past 3 months, such as a heart attack, stroke, or blood clot (confirm with trial site)
- Your A1c blood sugar level is below 7%
- You do not speak or read English or Spanish
- You are unable to give a blood sample
