Who might be able to join this trial:
- People aged 18 years or older
- People who have been diagnosed with Type 2 diabetes and/or high blood pressure
- People who attend regular follow-up appointments for Type 2 diabetes and/or high blood pressure at a Hospital Authority clinic
- People who have not been admitted to an emergency department or hospital in the past 3 months
- People whose medications have not been changed in the past 3 months
- People who are currently taking 5 or more ongoing medications
- People who are taking at least 1 higher-risk medication, such as blood thinners, diabetes tablets, insulin, mental health medications, or medications that suppress the immune system
- People who are able to hold a conversation in Cantonese and/or English
Who may not be able to join:
- People under the age of 18
- People who are not able to give their own informed consent to participate
- People who are not able to communicate in Cantonese and/or English
