Who might be able to join this trial:

  • You (or your parent or legal guardian) are willing and able to give written consent to participate
  • You are 9 years old or older
  • You have epilepsy that has not been controlled by medications, where the seizures start in one area of the brain, and either surgery is not an option, a previous surgery did not work, you have chosen not to have surgery, or surgery is planned but fits within the timeline of this study
  • You have previously had a brain monitoring procedure (called SEEG, where electrodes are placed in the brain) that clearly identified where your seizures are coming from, and all the required scan data from that procedure is available
  • You have specific brain scan images (a type of MRI and a CT scan taken during the SEEG procedure) available for use in this study
  • If you have a vagus nerve stimulator (VNS) device, it is either not helping or only partially helping, and its settings have not been changed for at least 6 months
  • You are able to have a research MRI scan suitable for the brain stimulation planning needed in this study
  • You are having 3 or more seizures per month during the lead-up period before treatment begins
  • Your epilepsy medications have not changed in the 4 weeks before the study begins (not counting emergency medications)
  • You are able to complete questionnaires and memory/thinking assessments, in the opinion of the study doctor
  • You can understand, speak, and write in French
  • You are able to follow all the steps and procedures required by the study
  • You are covered by a health insurance plan

Who may not be able to join:

  • You have had seizures that affect the whole brain (generalised onset seizures) in the past 12 months
  • Your seizures come from more than one area of the brain, from both sides of the brain, or from an area that is not clearly identified; the seizure source also cannot be limited only to certain specific regions at the front of the brain (confirm with trial site)
  • Your seizures are not caused by epilepsy but are psychological in nature (sometimes called non-epileptic episodes)
  • You cannot have a 3T MRI scan — for example, because you have a pacemaker, certain metal objects in your body, a non-removable electronic medical device, claustrophobia, or are unable to lie still; note that people with a VNS device are generally excluded from the MRI, unless they took part in specific previous trials and agreed to share their data (confirm with trial site)
  • You have a problem with substance misuse, including alcohol or drugs such as heroin, cocaine, or ecstasy
  • You have a serious ongoing illness or a brain tumour that is growing
  • You have damaged, broken, or irritated skin on your scalp (such as eczema or open sores) that could interfere with the brain stimulation treatment
  • You have metal implants in your skull, such as surgical clips or shrapnel (small titanium plates thinner than 1mm and dental fillings are generally acceptable), or you have certain medical devices such as a pacemaker, deep brain stimulator, medication pump, or cochlear implant
  • You have had previous skull surgery that left a hole or gap large enough to be a safety concern for the stimulation used in this study (confirm with trial site)
  • You have any other condition that the study doctor believes could make participation unsafe or affect the quality of the study results
  • You are pregnant, breastfeeding, currently detained by a court order, considered socially vulnerable in a way that affects your ability to consent, or are legally unable to give your own consent (confirm with trial site)
