Who might be able to join this trial:

• People aged 18 years or older
• People whose general health and ability to carry out daily activities falls within a certain range, as measured by a standard cancer performance scale (scores 0–3)
• People diagnosed with stage III or IV stomach cancer, esophageal cancer, liver, pancreatic or bile duct cancer, or lung cancer that cannot be removed by surgery or has spread to other parts of the body, confirmed by a tissue or cell sample
• People who are receiving chemotherapy aimed at managing their cancer rather than curing it, whether as a first or second course of treatment
• People who are expected to live for at least 3 months
• People who are able to swallow tablets or capsules by mouth and have no dietary restrictions that would prevent this
• People who are willing to follow the study schedule, attend follow-up appointments, and sign a consent form agreeing to take part

Who may not be able to join:

• People whose weight changes are caused by factors other than the cancer itself, such as fluid retention or fluid build-up in the abdomen
• People who have a mental health condition or who are currently taking antipsychotic or antidepressant medications
• People with a history of brain or nervous system conditions, such as brain metastases or epilepsy
• People who regularly take sedative (calming) medications, have been on long-term hormone therapy, have a history of ongoing heavy alcohol use, or are dependent on any substances or medications
• People who have a medical reason that makes taking the drug olanzapine unsafe for them
• People who have previously experienced a serious drop in white blood cells caused by a drug called clozapine, as this may increase the risk of a similar reaction with olanzapine
• People with diabetes that is not well controlled, or with seizures that are not well controlled
• People who are pregnant or breastfeeding
• People who are currently taking part in another clinical trial that involves a treatment or intervention, are involved in an observational study, or are still in the follow-up phase of a previous clinical trial
• People who have any other condition that the trial doctors believe could put them at risk or prevent them from completing the study requirements (confirm with trial site)
