Who might be able to join this trial:

• People who have given written consent to participate in the study
• People aged 18 up to (but not including) 75 years old, of any gender
• People whose cancer has been confirmed by pathology testing to be adenocarcinoma of the stomach or the junction between the stomach and oesophagus
• People whose tumour tests positive for HER2, based on specific laboratory results from either the primary tumour or a spread (metastatic) lesion
• People whose cancer has come back or spread and cannot be treated with surgery, who have at least one measurable area of cancer, and who are expected to live at least 12 weeks
• People with a good general health and activity level, rated 0 or 1 on a standard medical performance scale (confirm with trial site)
• People whose cancer has stopped responding to at least one prior course of treatment for advanced or metastatic disease
• People whose blood, liver, and kidney function test results meet the specific minimum levels required by the trial (confirm with trial site for exact values)
• Women who are able to become pregnant, and their partners, who are willing to use effective contraception during treatment and for 7 months after treatment ends

Who may not be able to join:

• People with a known allergy to the study drugs or their ingredients
• People whose heart pumping function (measured by a heart scan) falls below a specific threshold before the trial starts (confirm with trial site)
• People who have previously been treated with a type of cancer drug called an anti-HER2 ADC (antibody-drug conjugate)
• People who have received immunotherapy, biological therapy, or taken part in another clinical drug trial within the past 2 weeks
• People who have had surgery within the 3 weeks before starting the trial and have not fully recovered
• People with brain or nervous system spread of cancer that is not well controlled, or who need medication to manage seizures
• People with serious ongoing illness, such as active infection or diabetes that is not well controlled
• People who have had another type of cancer within the past 5 years, except for certain skin cancers or early-stage cervical cancer
• People with active heart conditions such as coronary artery disease, heart muscle disease, significant heart failure, severely high blood pressure, heart valve disease, or serious heart rhythm problems
• People who are currently taking long-term or high-dose steroid medications (short-term or inhaled steroids for nausea or appetite are generally permitted — confirm with trial site)
• People who are not legally able to give consent, or where medical or ethical reasons would prevent them from continuing in the study
• Women who are pregnant, breastfeeding, or planning to become pregnant during treatment
• People with fluid build-up around the lungs or in the abdomen that is not controlled
• People with an ongoing infection rated above a moderate level, wounds or fractures that are not healing, or a history of organ transplant
• People who still have side effects above a mild level from any previous cancer treatment (with the exception of hair loss, anaemia, or an underactive thyroid)
• People assessed by the trial investigators as not suitable to participate for any other reason
• People who are already taking part in another clinical trial at the same time
