Who might be able to join this trial:

For the main diet intervention (and related interviews):

  • Young people who have been diagnosed with type 2 diabetes (confirmed by a blood sugar level measurement, and not type 1 or a rare genetic form of diabetes).
  • Young people whose recent blood sugar control reading (HbA1c) falls within a specific range — not too low and not too high — including those on diabetes medication (confirm exact values with trial site).
  • Young people aged 12 to 17 years old.
  • Young people whose weight-for-height measurement (BMI) is at or above the 98th percentile for their age and sex, based on UK growth charts.
  • Young people aged 16–17 who can give their own consent, or young people aged 12–15 whose parent or carer gives consent on their behalf, with the young person also agreeing to take part.
  • Young people who are willing to follow and commit to a low-energy diet programme, including all phases and study visits.
  • For the interview part, young people and their parent or carer who are both willing to take part in a qualitative (experience-based) interview together.

For parents or carers (interview only):

  • A parent or carer of a young person who meets the eligibility criteria listed above.
  • Parents or carers who are able to give their own consent and are willing to take part in a qualitative interview alongside the young person.

For healthcare professionals (interviews only):

  • Registered healthcare professionals who have experience delivering this trial to adolescents and are willing to be interviewed about their experience, including any motivational interviewing training they have received.

Who may not be able to join:

  • Young people whose blood sugar control reading (HbA1c) is above a certain high level (confirm exact threshold with trial site).
  • Young people who have certain proteins in their blood (autoantibodies) that are associated with a different type of diabetes, as determined by their local clinic.
  • Young people with a confirmed genetic condition that causes obesity (such as a SIM1 gene mutation) or a syndrome linked to diabetes, such as Prader-Willi syndrome, Bardet-Biedl syndrome, or Wolfram's syndrome.
  • Young people whose diabetes developed as a result of bone marrow transplant or chemotherapy.
  • Young people with a significant mental health condition.
  • Young people who are pregnant, breastfeeding, or planning to become pregnant during the main study phases.
  • Young people with any other health condition that the study investigator considers would make the low-energy diet unsafe or unsuitable for them (confirm with trial site).
  • Young people who have taken part in another interventional clinical trial within the past 6 months.
  • Young people where the required consent and/or agreement to participate has not been given.
  • Young people who already have a diagnosed eye condition related to diabetes (retinopathy).
  • Young people who cannot consume any ingredient in the meal replacement products used in the study — including lactose — due to allergies, intolerances, religious reasons, or personal dietary choices.
  • Young people who have previously had scoliosis (spinal curvature) repair surgery.

There are no exclusion criteria listed for healthcare professionals.
