Who might be able to join this trial:

• People aged 18 to 75 years old
• People who have gastric cancer that has spread to the lining of the abdomen (peritoneum), confirmed by tissue or fluid testing during surgical exploration
• People whose cancer spread to the abdominal lining is limited (classified as P1 or P2 according to a specific gastric cancer staging system)
• People with liver spread where the largest affected area measures 5 cm or smaller
• People with certain lymph node spread in the area around the major abdominal blood vessel (the aorta), in a specific region between two named points, where the affected lymph nodes are at least 1 cm in size
• People who have ovarian tumours caused by the spread of gastric cancer (called Krukenberg tumours), in one or both ovaries — this counts as one site of spread
• People who have spread to no more than two sites in the body
• People who are in good general health and able to carry out daily activities without limitation (confirmed with trial site)
• People whose blood test results show the bone marrow, liver, and kidneys are working well enough to safely participate
• People whose expected survival is greater than 3 months
• People who are able to receive nutrition through a feeding tube and have the mental capacity to give informed consent
• People who have already completed 4 cycles of standard first-line systemic (whole-body) treatment for their cancer
• People for whom a repeat keyhole (laparoscopic) procedure after first-line treatment has shown that surgery to remove the cancer appears feasible
• People whose scans show no cancer spread to areas outside the abdomen (such as the lungs or bones)
• People whose scans show the abdominal lining disease has not grown or spread further since starting treatment
• People with a specific cancer spread score (called PCI) below 12
• People where only cancer cells in fluid are found, without visible spread on the abdominal lining, may still be considered
• People for whom surgeons believe there is a reasonable chance of removing all visible cancer with clear edges (called R0 resection)
• People for whom an appropriate level of lymph node removal during surgery is considered feasible

Who may not be able to join:

• People who have or develop cancer spread to areas outside the abdomen (such as lungs or bones) at any point
• People who have had another cancer diagnosed in the last 5 years, except for a specific type of non-melanoma skin cancer (basal cell carcinoma) or very early-stage cervical changes
• People of reproductive age who are not willing to use adequate contraception during the trial
• People with significant other health conditions that, in the opinion of the trial doctor, would make participation unsafe or unsuitable
• People with uncontrolled ongoing or active infections, heart failure, unstable chest pain, or irregular heart rhythm
• People with mental health conditions or personal circumstances that would make it difficult to follow the trial requirements
• People who are pregnant or breastfeeding
• People who have previously had surgery to remove part of the stomach for gastric cancer
• People whose cancer got worse — either on scans or based on symptoms — while receiving first-line treatment (including worsening fluid build-up in the abdomen that required medical procedures, or caused significant symptoms)
• People whose cancer is judged to be too extensive to remove safely, including those with a PCI score above 12, large sections of bowel involved, cancer in certain parts of the pancreas or bile ducts, cancer in a specific area near the liver called the hepatoduodenal ligament, cancer wrapped around major blood vessels, or cancer spread into the food pipe (oesophagus) in a way that makes complete removal unlikely
• People who choose to withdraw from the study at any time
• People for whom the trial doctor decides it is in their best interest to stop participation
