Who might be able to join this trial:

  • People aged 18 to 64 with a blood sugar control reading (called A1C) above 7, or people aged 65 and older with an A1C above 8, measured within the past 12 months before the study starts.
  • People aged 18 to 64 with an A1C above 7, or people aged 65 and older with an A1C above 8, at the time the study begins.
  • People who receive Medicaid, or both Medicaid and Medicare.

  • People who can use their smartphone for things beyond just emailing, texting, and making phone calls (this will be assessed over the phone based on self-reported ability).

Who may not be able to join:

  • People who are currently pregnant.
  • People who have cognitive impairment (difficulties with memory or thinking), which will be assessed using a short screening tool called the Six-Item Screener.
