For young people (ages 12–18):

  • People who receive care for type 2 diabetes at either CHLA or UCSF
  • People who speak English
  • People who have been prescribed medication for type 2 diabetes
  • People between 12 and 18 years old at the time of joining the trial
  • People willing to attend clinic visits, take part in group sessions (if assigned to the Team Clinic group), and complete surveys

For parents or caregivers:

  • People who have a child aged 12 to 18 who receives care for type 2 diabetes in the endocrinology clinic at CHLA, and whose child has agreed to take part as a youth participant

  • People who are non-verbal or unable to take part in group activities and discussions
  • People who are pregnant
  • People whose parent, guardian, or legal representative is unable or unwilling to provide written consent, or who are themselves unable or unwilling to provide written agreement to participate
