Who might be able to join this trial:
- You are 60 years old or older at the time of screening
- You have been diagnosed with type 2 diabetes
- You are currently taking two or more diabetes pills (not insulin), and your medication has stayed the same for at least the past 3 months

- The trial doctors feel you are not suitable to participate — for example, if you have dementia, certain mental health conditions, or severe vision or hearing difficulties that would make it hard to use the monitoring device
- You have surgery or a medical procedure planned in the next 6 months that might affect your ability to attend follow-up visits
- You are currently using or expect to use steroid medications (such as cortisone, taken by mouth, injection, or drip) that could affect your blood sugar levels
- Your kidneys are not working well enough, based on a specific kidney function measurement being too low (confirm details with trial site)
