Who might be able to join this trial:
- People aged 18 or older
- People who have been diagnosed with type 2 diabetes and are scheduled for surgery lasting 2 hours or more, where a hospital stay of more than 72 hours is expected, along with short-term nutrition delivered through a drip (intravenous nutrition) around the time of surgery
- People whose blood sugar is not well controlled, meeting at least one of the following: a long-term blood sugar measure (HbA1c) of 7.5% or higher, or a random blood sugar reading of 13.9 mmol/L or higher; OR type 2 diabetes that remains poorly controlled (HbA1c 7.5% or higher) despite taking two or more diabetes tablets; OR people already on insulin whose blood sugar remains above target (HbA1c 7.0% or higher) even after dose adjustments
- People who are willing to sign a consent form agreeing to take part
Who may not be able to join:
- People experiencing a serious sudden diabetes complication, such as diabetic ketoacidosis or a dangerous high blood sugar emergency
- People with type 1 diabetes or another specific type of diabetes other than type 2
- People with serious problems affecting the heart (heart failure classified as Class III or above), liver (certain elevated liver enzyme levels), kidneys (kidney filtration rate at or below 30 mL/min/1.73 m²), low red blood cell levels (haemoglobin below 90 g/L), low white blood cell or platelet counts, or unstable blood pressure and circulation
- People with other hormonal conditions that affect blood sugar levels, such as an overactive or underactive thyroid, or Cushing's syndrome
- People with a known allergy or sensitivity to any medications or materials used in this trial
- People for whom standard insulin pump therapy is not medically appropriate
- People with skin conditions such as rashes or itching disorders, or with blood clotting problems
- People who have reduced consciousness or a mental health condition that affects their ability to make decisions or communicate
- People who develop serious complications from surgery
- People considered unsuitable for the trial by the research team for other reasons (confirm with trial site)
