Who might be able to join this trial:
- People between 18 and 75 years old
- People who have been diagnosed with Type 2 diabetes
- People whose HbA1c blood sugar level is 7.5% or higher (a measure of average blood sugar over the past few months)
- People who take at least one insulin injection per day
- People who are already a patient at a primary care, endocrinology, or diabetes clinic in New Orleans or the surrounding area
- People who have Medicaid, free care, or no health insurance
- People who can speak and understand English and are able to give their agreement to take part in the study
Who may not be able to join:
- People who have Type 1 diabetes
- People who are currently using a continuous glucose monitor (CGM) or an insulin pump
- People with advanced kidney disease or low kidney function (eGFR below 40) (confirm with trial site)
- People with serious other health conditions that, in the opinion of the study doctor, would make it difficult to take part
- People who have been diagnosed with end-stage kidney disease, are on dialysis, have had a kidney transplant, have certain blood conditions affecting red blood cells (called hemoglobinopathies), are on iron therapy, or have any other condition that affects how the HbA1c test is measured
- People who are pregnant
