Who might be able to join this trial:

• You are willing to take part voluntarily and sign a consent form
• You are 18 years old or older (any gender)
• You have been diagnosed (through a tissue or cell sample) with a type of stomach or gastroesophageal junction cancer called adenocarcinoma that cannot be surgically removed, has spread locally, or has spread to other parts of the body — and a specific protein marker in your blood (alpha-fetoprotein) was above a certain level at diagnosis
• You have at least one tumour that can be measured on a scan, meeting specific size requirements
• Your cancer got worse during or after a first-line treatment combining a PD-1 immunotherapy drug with chemotherapy; or your cancer came back within 6 months of finishing a certain type of chemotherapy after surgery, provided that treatment also included a PD-1 inhibitor
• Your general health and ability to carry out daily activities meets a certain standard (confirm with trial site)
• You are expected to live for at least 12 weeks
• Your blood counts, liver function, kidney function, and other organ functions are within acceptable ranges based on recent blood tests (no blood transfusions or certain growth factor medicines in the 14 days before testing)
• If you are a woman who could become pregnant, you are using effective contraception
• You are willing and able to attend follow-up visits and follow the study plan

Who may not be able to join:

• You are unable or unwilling to follow the study plan and procedures
• Your cancer is known to be HER2-positive (a specific protein found on cancer cells)
• Your stomach cancer is a different type, such as squamous cell carcinoma, undifferentiated carcinoma, or a mixed type that includes other tissue types
• You currently have any condition that would affect how your body absorbs the study medicines
• You have serious heart or cardiovascular problems, such as uncontrolled heart failure, coronary heart disease, irregular heartbeat, or uncontrolled high blood pressure
• You have active cancer that has spread to the brain — though people with a small number of very small, symptom-free brain metastases may still be eligible in certain circumstances (confirm with trial site)
• You are known to be allergic to any of the medicines used in this trial
• You have previously been treated with certain immune-boosting medicines other than anti-PD-1/PD-L1 drugs (confirm with trial site)
• You had significant bleeding, a stomach or bowel perforation or fistula, or a blood clot event (such as a stroke, deep vein thrombosis, or pulmonary embolism) within a recent period before joining
• You have an active, uncontrolled infection such as severe pneumonia, or active hepatitis B or C
• You have a known history of a condition where your immune system does not work properly from birth, or you have active tuberculosis
• You have had an organ transplant or a stem cell transplant from a donor
• You are known to be HIV positive
• You have had significant unintentional weight loss or a major drop in food intake recently, unless this has been corrected for more than four weeks before starting the study medicine
• You have had another type of cancer in the past, unless it was fully treated with no signs of return for at least 2 years, was a certain type of treated skin cancer with no recurrence, or was a non-invasive cancer (carcinoma in situ) that was fully treated with no signs of return
• You are pregnant or breastfeeding
• You have other health conditions that the research team believes could put you at serious risk or prevent you from completing the study
• The research team considers you unsuitable for the trial for any other reason
