Who might be able to join this trial:
- People who have been diagnosed with type 2 diabetes
- People who also have at least one of the following conditions: blocked or narrowed arteries in the heart (coronary artery disease), blocked or narrowed arteries in the legs or arms (peripheral artery disease), heart failure, kidney disease related to diabetes (confirmed by specific kidney function test results for at least three months), eye damage related to diabetes (diabetic retinopathy), a previous stroke, an irregular heart rhythm called atrial fibrillation or atrial flutter
- Alternatively, people aged 55 or older who have at least two of the following: being a current smoker, taking blood pressure medication or having high blood pressure readings, taking cholesterol-lowering medication or having elevated cholesterol levels, or having a body mass index (BMI) over 25
Who may not be able to join:
- People whose kidneys are functioning at a very low level (eGFR below 30 — confirm with trial site)
- People with a high level of protein in their urine (uACR above 200 — confirm with trial site)
- People with a BMI over 50
- People whose heart is pumping at a very low capacity (ejection fraction below 30% — confirm with trial site)
- People who currently have cancer, or who have finished cancer treatment within the last five years
- People who are blind in both eyes
- People who have severe psychotic depression
- People who are not able to use apps on a digital device such as a smartphone or tablet
