Who might be able to join this trial:
- People who are overweight or obese (with a Body Mass Index between 30 and 45) who have had type 2 diabetes for less than 6 years or for more than 10 years, and whose diabetes is managed through diet alone or diet combined with tablets (not insulin)
- People who are overweight or obese (BMI between 30 and 45) whose blood sugar levels are higher than normal but not yet in the diabetes range — sometimes called "pre-diabetes"
- People who are overweight or obese (BMI between 30 and 45) who do not have diabetes, included as a comparison group
- People aged between 45 and 79 years old
- Women who have been through menopause
- People who communicate well in English and are able to give informed consent and follow dietary guidance
- People who are willing and able to follow the study plan, including dietary changes and attending scheduled visits
Who may not be able to join:
- People currently using insulin
- People whose long-term blood sugar control (measured by an HbA1c blood test) is very high — above 12% (confirm with trial site)
- People who have lost more than 5 kg in weight in the past 6 months
- People who have had a heart attack within the last 6 months
- People with a known cancer diagnosis in the past 5 years
- People without diabetes who have a parent, sibling, or child with type 2 diabetes (as this affects the non-diabetic comparison group)
- People with a history of diabetes during pregnancy (gestational diabetes)
- People who are unable to have an MRI scan — for example, due to metal implants in the body or severe claustrophobia
- People who drink more than 14 units of alcohol per week
- People with advanced kidney or liver disease
- People currently taking steroid medications or antipsychotic medications
- People currently taking part in another clinical trial
- People whose life expectancy is less than one year
- People with an allergy to local anaesthetic — this applies to those taking part in the tissue sample (biopsy) part of the study
- People with any other condition that the trial team considers may affect their safety or ability to follow the study plan (confirm with trial site)
