Who might be able to join this trial:

• Adults aged 18 or older, any gender
• People whose general health and ability to carry out daily activities falls within an acceptable range (scored 0–2 on a standard oncology scale), with a life expectancy of more than 3 months
• People who have been diagnosed with stomach cancer that has spread to other parts of the body or to nearby tissues, confirmed by a laboratory test of cells or tissue
• People whose recent blood and organ function test results (taken within 7 days before screening) meet specific minimum levels for white blood cells, platelets, haemoglobin, liver enzymes, protein, and kidney function (confirm exact values with the trial site)
• People who have at least one measurable area of cancer that can be tracked on scans, as assessed using standard medical criteria
• People who have not yet received chemotherapy for advanced disease, and who are planned to receive a first course of treatment combining immunotherapy (one of several approved medicines, chosen by the treating doctor) with chemotherapy (one of several approved combinations, chosen by the treating doctor)
• People (or their legal representative or guardian) who are willing to sign a consent form confirming they understand the study and agree to take part

Who may not be able to join:

• Women who are pregnant or breastfeeding, or women of childbearing age who have not used adequate contraception
• People who have had another type of cancer in the past 5 years, except for certain previously cured skin cancers or cervical pre-cancers
• People who have a brain tumour or cancer that has spread to the brain and is not currently under control, or who have significant pressure in the skull or neurological or psychiatric symptoms
• People with serious or uncontrolled heart conditions — including heart attack, heart failure, unstable chest pain, significant fluid around the heart, or unstable heart rhythm — occurring within the 6 months before joining
• People with serious neurological or psychiatric conditions such as dementia or epilepsy
• People with severe or uncontrolled infections, or serious blood clotting problems, or a clear tendency to bleed
• People with significant damage to major organs
• People with a build-up of fluid around the lungs or in the abdomen that is causing breathing difficulty rated at a certain level of severity, and that requires direct local treatment
• People who have been taking probiotics or antibiotics on a long-term basis
• People whom the trial investigator considers unsuitable for the trial for any other reason
