Who might be able to join this trial:
- People aged between 20 and 80 years old
- People who are willing and able to understand and agree to the study procedures
- People who do not have severe emotional or consciousness problems before the procedure that would prevent them from taking part in study tasks
- (For the temporary brain monitoring group) People who have a brain tumour in or near important brain areas, or difficult-to-control epilepsy, and are already scheduled for an awake brain surgery as part of their standard care

- (For the permanent implant group) People who have severe difficulty speaking or using language due to stroke, brain tumour, ALS (motor neurone disease), or locked-in syndrome
- (For the permanent implant group) People whose condition has not improved substantially after 3 to 6 months of proper rehabilitation, and whose condition has lasted more than 12 months
- (For the permanent implant group) People whose brain areas involved in imagining or planning speech are still structurally intact (confirm with trial site)
- (For the permanent implant group) People diagnosed with severe difficulty producing speech or severe difficulties with language, with a very low score on a standardised Chinese language test, but whose ability to understand spoken language remains at or above 80% of normal levels
Who may not be able to join:
- People with severe pressure inside the skull that would make awake brain surgery or electrode placement unsafe
- People with severe brain or nervous system problems that would stop them from taking part in study procedures (note: this does not apply to speech or language difficulties in the permanent implant group)
- People who cannot have an MRI scan, or who cannot undergo awake brain surgery — for example due to certain implanted medical devices, severe claustrophobia, or a breathing condition called obstructive sleep apnea
- People with severe psychiatric illness or significant memory and thinking difficulties (based on a standard cognitive test score below 24)
- People with serious health conditions affecting other parts of the body
- People who are pregnant or breastfeeding
- People who are unwilling or unable to give informed consent
