Who might be able to join this trial:
- You have a brain tumor (medulloblastoma, ependymoma, ATRT, or another central nervous system tumor) that has come back or continued to grow after previous treatment.
- Your tumor type was confirmed by a biopsy or tissue sample, either when first diagnosed or when the tumor came back.
- For one group (Stratum IV), the medulloblastoma must also be confirmed by a specific molecular test called methylation analysis (confirm with trial site).
- You are female or male, and were under 20 years old at the time of your original diagnosis.
- Your liver, kidneys, and blood cell counts are within acceptable ranges as tested by blood tests (confirm with trial site for specific values).
- You are well enough to carry out at least some daily activities, as measured by a standard health scoring scale (a score of 50 or above out of 100, or an equivalent child-friendly scale for those under 12).
- You (and/or your parent or legal guardian) have given written agreement to take part in the trial.
Who may not be able to join:
- You rely on a VP shunt or subdural peritoneal shunt to drain fluid from your brain (though you may still be considered for a different group in the trial — confirm with trial site).
- You have previously been treated with the drugs temozolomide or irinotecan (though you may still be considered for a different group — confirm with trial site).
- You currently have an active infection.
- You are pregnant or breastfeeding.
- You have already started receiving treatment specifically for your current relapse (note: surgery before starting this trial may still be allowed — confirm with trial site).
- You have a known allergy to any of the medicines used in this trial.
- You currently have an active stomach ulcer.
- You have a significant heart or blood pressure problem that is not controlled by standard treatment.
- You are expected to need major planned surgery during the course of the trial.
- You have any other medical condition that would make it unsafe to receive the trial treatment (confirm with trial site).
- You have a surgical wound that has not healed properly.
- You have a broken bone that has not healed properly.
