Who might be able to join this trial:

• People aged 18 or older
• People who have a confirmed solid tumour (confirmed by tissue or cell testing), or who have a known or suspected cancer that has spread, with a brain tumour that doctors believe is a brain metastasis (cancer that has spread to the brain) based on clinical and imaging findings
• People for whom a brain surgeon has recommended surgical removal of one brain metastasis, and who are considered medically suitable for surgery
• People for whom doctors have planned a targeted radiation treatment (called stereotactic radiosurgery) for all known brain lesions that need treatment, with no more than 10 metastases in total
• People who are reasonably active and able to carry out daily activities (based on a standard medical scale called ECOG, with a score of 2 or lower — confirm with trial site what this means for a specific situation)
• People who are able to give written consent to participate, or who have a legally authorised representative who can provide consent on their behalf
• People who are willing to attend follow-up visits at the trial site or with a local doctor, and who can arrange for clinical notes and scan images to be shared with the enrolling site if follow-up happens elsewhere

Who may not be able to join:

• People who are pregnant or currently breastfeeding
• People of childbearing potential who are not willing to use adequate contraception during the trial
• People with serious ongoing illnesses or other severe conditions that, in the investigator's judgement, would make participation inappropriate or interfere with the assessment of the treatment
• People with weakened immune systems, or people who are HIV-positive and currently receiving antiretroviral (HIV) medication — note: people who are HIV-positive but do not show signs of a weakened immune system may still be eligible (confirm with trial site)
• People with uncontrolled ongoing illness, such as an active infection, heart failure, unstable chest pain, irregular heart rhythm, or mental health or social circumstances that would make it difficult to follow the study requirements
• People who have previously had open brain surgery for cancer
• People whose cancer is known or suspected to be a germ cell tumour, small cell carcinoma, or lymphoma
• People who have previously had whole brain radiation therapy
• People with a known allergy to gadolinium (a contrast dye used in MRI scans), or who have a pacemaker or metal implant that is not safe for MRI — note: people with MRI-compatible implants, including MRI-compatible pacemakers, may still be eligible
• People who have cancer that has spread along the lining of the brain or spinal cord (called leptomeningeal disease)
• People who have a brain metastasis located 5 mm or closer to a specific part of the visual pathway called the optic chiasm
• People who have any single brain metastasis larger than 5 cm
• People who have more than 10 brain metastases
• People for whom surgery on 2 or more brain metastases is being recommended
• People who are expected to need high-dose steroid treatment (at a certain level — confirm with trial site) or a drug called bevacizumab for longer than 4 weeks
• People who are currently enrolled in another clinical trial studying radiation or surgical treatments for brain metastases
