Who might be able to join this trial:
- Adults aged 18 or older who have a confirmed diagnosis of chronic kidney disease (CKD) that is not being treated with dialysis, with kidney function or protein levels in the urine that meet specific medical thresholds (confirm with trial site)
- People who live in remote areas within the Peace River region of Alberta, Canada
- People who have been diagnosed with high blood pressure by a doctor, or who report having it and are currently taking medication to manage it

- People with very advanced CKD (the most severe stage, stage 5) or those already receiving kidney replacement therapy such as dialysis or a transplant
- People who have heart failure where the heart's pumping ability is significantly reduced
- People with a terminal illness where life expectancy is less than one year
- People currently taking part in another clinical drug trial
- People who are pregnant or breastfeeding
- People who are planning to move out of the Peace River region, or who live in an area without mobile phone coverage
