Who might be able to join this trial:
- People who have high levels of protein in their urine (above a certain threshold, measured by a urine test) OR have significantly reduced kidney function (stage 4 or 5 chronic kidney disease), AND
- People who are showing signs of fluid build-up in their body — such as swelling in the legs, fluid in the abdomen, or fluid around the lungs — and whose current body weight is at least 5 pounds above their estimated normal "dry" weight (the weight they would be at without that excess fluid)
Who may not be able to join:
- People who weigh less than 100 pounds or more than 300 pounds
- People who have a condition affecting their nervous system that causes their blood pressure to drop when they stand up
- People with low potassium levels in their blood at the time of enrolment
- People with moderately or severely low sodium levels in their blood at the time of enrolment
- People with creatinine levels in their blood above a certain threshold, or significantly elevated compared to their usual baseline level (confirm with trial site)
- People who are unable or unwilling to regularly check and record their blood pressure and weight at home
- People who do not have a working phone number or a smartphone
- People for whom doctors expect dialysis will be needed within 3 months
- People with a life expectancy of less than 6 months
- People living with a condition that would make it difficult to use the trial's digital tool, such as dementia, being unable to read, or blindness
- People who are pregnant
- People who are currently incarcerated
