Who might be able to join this trial:
- People who are transgender and are currently receiving cross-sex hormone therapy, or people who are acting as a comparison group (controls)
- People who are able to give informed consent; if the person is under 18 years old, all legal guardians must also give consent
- For one specific part of the study (Part B) only: People whose weight has been stable (within 5%) for the last 3 months and who have a body mass index (BMI) of 30 or below
Who may not be able to join:
- People with serious medical conditions, such as uncontrolled heart disease, severe heart failure, uncontrolled high blood pressure, a history of stroke, or active cancer
- People who started cross-sex hormone therapy on their own before the study begins, without medical supervision
- For Part B only: People with poorly controlled hormone or gland disorders, such as Cushing's disease, uncontrolled thyroid conditions, or other uncontrolled pituitary gland disorders
- People with ongoing lung conditions — such as chronic obstructive pulmonary disease — that would make it difficult to follow the study procedures; people with obstructive sleep apnea are also excluded. People with mild or exercise-triggered asthma may still be considered, provided they only use certain inhaler medications and have not needed them in the week before the study (confirm with trial site)
- People with diagnosed gut or digestive diseases, such as Crohn's disease, ulcerative colitis, coeliac disease, or an active stomach ulcer; people with gastro-oesophageal reflux may still be considered (confirm with trial site)
- People with a history of HIV infection or any ongoing long-term infection such as tuberculosis
- For Part D only: People who are unable to have an MRI scan — for example, because they have metal implants in their body, tattoos on the head or neck, or experience claustrophobia (fear of enclosed spaces)
