Who might be able to join this trial:

• People aged 18 or older
• People who speak English or Spanish
• People who are willing to follow all study procedures and take part for the full duration of the study
• People who are receiving cancer treatment at one of these specific hospitals or cancer centers: UCHealth Aurora, UCHealth Highlands Ranch, UCHealth North, UCHealth Memorial Hospital, UCHealth Parkview Hospital, San Juan Cancer Center, RMCC-Pueblo, SCL-St. Mary's, or Parkview Medical Center
• People who live in a rural county (as defined by Rural-Urban Continuum Codes 4–9) served by one of the participating hospitals or cancer centers listed in the trial
• People who have been diagnosed with lung cancer (including small cell or non-small cell lung cancer, or other lung cancer types)
• People who have been diagnosed with head and neck cancer (including cancers of the mouth, throat, voice box, or salivary glands, or other head and neck cancer types)
• People who have been diagnosed with thyroid cancer
• People who have been diagnosed with breast cancer
• People who have been diagnosed with cervical cancer
• People who have been diagnosed with colon or rectal cancer
• People who have been diagnosed with bladder cancer
• People who will be receiving surgery, radiation therapy, chemotherapy, or a combination of these treatments for their cancer, including treatment given before or after the main treatment
• People who live in a rural area AND also live in an area officially recognised as having a shortage of health professionals or being medically underserved, AND/OR who have no health insurance (public or private), AND/OR who have public insurance (such as Medicaid, Medicare Part B only, or VA coverage) or spend 10% or more of their annual income on out-of-pocket medical costs

Who may not be able to join:

• People under 18 years old
• People who do not speak English or Spanish
• People who are not receiving cancer treatment at one of the participating hospitals or cancer centers listed in the trial
• People who have been diagnosed with a type of cancer not listed in the inclusion criteria above
• People whose cancer type is listed in the inclusion criteria but whose treatment will not take place at one of the participating hospital sites
• People who have already started curative treatment for their current cancer diagnosis before joining the trial
• People in certain vulnerable situations at the time of joining, including those who are currently incarcerated or on probation, homeless, or pregnant (confirm with trial site)
• People who have a cognitive or personality condition, are experiencing suicidal thoughts, or are under the influence of alcohol or drugs at the time of consent, in a way that would affect their ability to take part in the study
• People who are unable to hear without an auditory aid (note: people who can hear with a hearing aid are not excluded)
• People who are unlikely to be able to be contacted over time, such as those with no permanent address at the time of joining

Note: People who become homeless, pregnant, lose their hearing, or lose their permanent address after they have already joined the trial may be allowed to continue participating until the study is complete (confirm with trial site).
