Who might be able to join this trial:

• People aged 18 to 75 years old
• People who have been diagnosed with nasopharyngeal carcinoma (a type of cancer in the back of the nose and throat) confirmed by laboratory testing of tissue or cell samples
• People whose cancer has spread to other parts of the body (metastatic), confirmed by scans or pathology, and classified as Stage IVB
• People whose cancer has spread in a limited way — specifically to no more than 5 spots in the body, across no more than 2 different organs (called "oligometastatic")
• People who have not yet received any treatment for their metastatic cancer
• People who the treating doctor has determined are not suitable for, or do not wish to receive, chemotherapy
• People who are generally well enough to carry out daily activities with little or no limitation (based on a standard medical scoring system called ECOG, scores 0–1)
• People with an estimated life expectancy of at least 6 months
• People who have at least one tumour spot that can be measured on a scan, according to standard criteria (RECIST v1.1)
• People whose blood, kidney, and liver test results fall within acceptable ranges in the 7 days before joining the trial (confirm with trial site for specific values)
• People who are willing and able to follow the study procedures and have signed a consent form

Who may not be able to join:

• People who have a known allergy to any ingredient in the study drug toripalimab
• People who have an active autoimmune disease, or a history of one that may return — though some conditions (such as controlled type 1 diabetes, well-managed underactive thyroid, controlled coeliac disease, or certain skin conditions not needing systemic treatment) may still allow participation (confirm with trial site)
• People who have had another active cancer within the past 2 years, apart from certain fully treated, low-risk cancers such as some skin cancers, early bladder cancer, or early-stage cervical or breast cancer
• People who have taken corticosteroid medications (such as prednisone at 10 mg per day or more) or other medicines that suppress the immune system within the 14 days before joining
• People with uncontrolled diabetes, or abnormal levels of potassium, sodium, or calcium that have not responded to standard treatment, or severely low protein levels in the blood in the 14 days before joining
• People with a history of certain lung conditions, such as interstitial lung disease, non-infectious pneumonia, or pulmonary fibrosis, or other uncontrolled conditions including high blood pressure
• People with a serious ongoing infection requiring antibiotics or antiviral treatment in the 14 days before joining
• People with a history of HIV infection
• People for whom immunotherapy or radiation therapy is medically inadvisable
• People with untreated chronic hepatitis B, or hepatitis B virus levels above a certain threshold, or active hepatitis C — though people with well-managed or cured hepatitis B or C may still be considered (confirm with trial site)
• People who have had surgery requiring general anaesthesia within the 28 days before joining
• People who have previously had a stem cell transplant or organ transplant from a donor
• People with certain heart or blood vessel conditions, including: chest pain from heart causes, a blood clot in the lungs, a heart attack, severe heart failure, significant heart rhythm problems, or a stroke — all within defined time periods before joining (confirm with trial site for specific timeframes)
• People with moderate to severe nerve damage in the hands or feet (grade 2 or above on a standard medical scale)
• People with other medical conditions or abnormal test results that could interfere with taking the treatment safely, interpreting side effects, or following the study reliably — including alcohol or drug dependence
• People already taking part in another interventional clinical trial at the same time
