Who might be able to join this trial:
- You have been diagnosed with OCD as your main current condition
- You are 18 years of age or older
- Your OCD symptoms score 16 or higher on a standard OCD severity scale called the YBOCS (confirm with trial site)
- You have not responded well enough to a standard OCD therapy called exposure and response prevention (ERP), or you had to stop ERP because anxiety or avoidance was too overwhelming
- You have other conditions alongside OCD — such as depression, anxiety disorders, ADHD, tic disorders, eating disorders, personality disorders, or autism spectrum disorder (when it is not your main diagnosis) — and these are still permitted
- You are willing and committed to taking part in intensive therapy sessions, including practice at home on your own
- You are either not taking any psychiatric medication, or you have been on a stable dose of an antidepressant (such as an SSRI, SNRI, or clomipramine) or certain other psychiatric medications for at least 8 weeks
- You are able to attend treatment sessions 4 days per week for 5 to 7 weeks at one of the participating sites near your home or work
- You are able to attend a brain scan (MRI) session at one of the academic trial sites near your home or work
- You are able to give informed consent to take part in the trial
Who may not be able to join:
- Your main OCD symptom is hoarding
- You currently have a diagnosis of a psychotic disorder, bipolar disorder, autism spectrum disorder (when it is your main diagnosis), or a substance use disorder
- You are currently having active thoughts of suicide with intent to act on them
- You regularly use benzodiazepine medications (such as diazepam or lorazepam)
- You have a cochlear implant (a device implanted in the ear)
- You have epilepsy, or have had it in the past
- You are currently pregnant
- You have severe claustrophobia or have metal objects in or on your body that would prevent you from having an MRI scan
- An MRI scan has shown a growth or mass in your brain
- You have previously received a treatment called rTMS (a type of brain stimulation)
