Who might be able to join this trial:
- You are 18 years of age or older
- You have a half-matched (haploidentical) family donor available — this means a donor who shares about half of their tissue type markers with you, such as a parent, child, or sibling
- You have sickle cell disease (SCD) that is considered severe, meaning at least one of the following applies to you:
- You have had a stroke or a serious neurological event lasting more than 24 hours
- You have had 2 or more acute chest crises within a 2-year period, even while receiving the best available treatment
- You have had 3 or more severe pain crises per year over a 2-year period, even while receiving the best available treatment
- You need regular blood transfusions (8 or more per year) to prevent complications
- You need transfusions but have developed antibodies that make it very difficult to find compatible blood, or you have a very rare blood type
- You need hydroxyurea or transfusions for your SCD but cannot tolerate either treatment due to serious side effects
- You have lasting damage to organs caused by SCD, such as blood vessel or liver problems (confirm with trial site)
- Your general health is good enough to safely undergo a stem cell transplant, including your heart, lungs, kidneys, and liver all meeting certain minimum levels of function (confirm specific thresholds with trial site)
- You are able and willing to give written consent to take part in the trial
Who may not be able to join:
- You have a fully matched sibling donor available
- You have already had a bone marrow transplant in the past
- You are currently pregnant or breastfeeding
- You are able to father or become pregnant and are not willing to use effective contraception during the trial
- You have significant antibodies in your blood that are directed against your donor's tissue type, which could cause problems with the transplant (confirm with trial site)
- You have HIV, or currently active Hepatitis B or Hepatitis C infection
- You have an infection (bacterial, fungal, or viral) that is not currently under control
- You have taken part in another clinical intervention trial within the last three months
- You have another existing health condition that your doctor believes would significantly increase the risks of having this type of transplant
