For patients (main study):
- You are between 50 and 77 years old
- You are a current smoker, or you used to smoke and quit within the last 15 years
- You have smoked the equivalent of at least 20 "pack-years" (for example, one pack a day for 20 years, or two packs a day for 10 years) (confirm with trial site)
- You have a regular (non-emergency) appointment scheduled at one of the study clinics

For online surveys (healthcare providers):
- You are a primary care provider or navigator at one of the participating clinics

Who may not be able to join:
For patients (main study):
- You do not speak English
- You have a personal history of lung cancer
- You have already had a lung cancer screening scan in the past 12 months
- Your doctor has determined that certain health conditions make you a poor candidate for treatment
- You are unable to give informed consent to participate
For healthcare staff (interviews and surveys):
- You are unable to give informed consent to participate
- You are currently pregnant
