Who might be able to join this trial:

• People who are able to give written consent and are willing to follow the study schedule and requirements
• People who are 21 years of age or older at the time of signing the consent form
• People who fall into one of these three groups: (1) suspected nasopharyngeal cancer (a type of cancer at the back of the nose/throat) based on symptoms such as neck swelling, nosebleeds on one side, or blocked nose; (2) newly diagnosed, lab-confirmed nasopharyngeal cancer at a mid-to-advanced stage (Stages 2 to 4A); or (3) newly diagnosed nasopharyngeal cancer that has returned or spread to distant parts of the body
• People whose cancer has been confirmed to be linked to a virus called Epstein-Barr virus (EBV), shown through lab tests on the tumour or through blood tests
• People who have had the required scans to determine the extent of the cancer, including an MRI or CT scan of the head and neck area, and scans of the chest, abdomen, and pelvis (or a full-body PET-CT scan)
• People whose general physical health and ability to carry out daily activities meets a standard level of fitness for the trial (confirm with trial site)
• People whose heart, liver, kidneys, and other organs are functioning at an adequate level (confirm with trial site)

Who may not be able to join:

• People who are under 21 years of age or 100 years of age or older
• People who have previously received radiation therapy or any anti-cancer treatment for nasopharyngeal cancer, including experimental treatments, that are not part of the planned treatment in this trial
• People who have cancer that has spread to the brain or the lining around the brain or spinal cord
• People who have had another active cancer within the past 2 years, with some exceptions for certain early-stage or fully treated skin or other localised cancers (confirm with trial site)
• People who have serious ongoing or active infections that require treatment with antibiotics, antifungal, or antiviral medications, including tuberculosis, or who have been hospitalised for a serious infection within 4 weeks before joining the trial
• People with untreated or unstable hepatitis B, or hepatitis B with a viral level above a certain threshold, or active hepatitis C infection — though people with stable or cured hepatitis B or C may still be considered (confirm with trial site)
• People who have previously had a stem cell transplant or an organ transplant from another person
• People who have had certain serious heart or blood vessel problems within the past 28 days, such as cardiac chest pain, a blood clot in the lungs, a stroke, or a seizure
• People who have had a severe allergic reaction to any of the chemotherapy medicines used in this trial (gemcitabine, cisplatin, or capecitabine)
• People who have used herbal medicines intended to treat cancer within 14 days before joining the trial
• People who still have unresolved side effects from a previous cancer treatment that have not returned to their normal baseline level
• People with other significant medical conditions, or issues with alcohol or drug dependence, that could interfere with treatment or make it difficult to assess side effects or follow the study requirements
• People who are currently taking part in another clinical trial involving a treatment
