Who might be able to join this trial:
- You score 25 or higher on a standard OCD severity scale called the Yale-Brown Obsessive Compulsive Scale.
- You have tried at least two different medications from the SSRI (serotonin reuptake inhibitor) family, each at the highest recommended dose (if you could tolerate it), for at least 10 weeks each, and they did not help enough.
- You have also tried at least one older type of antidepressant called a tricyclic medication, at the highest recommended dose (if tolerated), for at least 10 weeks, without enough improvement.
- You have tried at least one additional medication added on top of another treatment, such as an antipsychotic, lithium, a calming medication (benzodiazepine), or buspirone.
- You have completed at least 20 sessions of talking therapy, such as Cognitive Behavioral Therapy (CBT) or a similar approach.
- You are able to read, understand, and sign a consent form agreeing to take part in the study.
Who may not be able to join:
- You have ever had, or currently have, a condition involving psychosis (losing touch with reality).
- You have had a manic episode (an extreme high mood episode) within the past three years.
- You have a significant medical or neurological condition that would make brain stimulation surgery (DBS) unsafe for you.
- You have a condition that affects how your body processes sugar, such as diabetes.
- Your pre-surgery brain scan (MRI) shows a significant abnormality, or you are unable to have an MRI scan.
- You currently have, or have recently had, problems with substance misuse or dependence (not including nicotine/smoking).
- You are pregnant, planning to become pregnant during the study, or unwilling to use a reliable method of contraception.
- You currently have a severe personality disorder (except for a specific type called anankastic personality disorder, which may still be allowed).
- You are unable or unwilling to follow the study's procedures and requirements.
- You are currently experiencing serious, unstable thoughts of suicide.
