"Who might be able to join this trial:
- The trial involves primary care practices (such as family medicine, geriatrics, or general internal medicine) that are part of one of the 4 health systems participating in the study.

- People who are able to communicate in English or Spanish may be eligible.
Who may not be able to join:
- People who have had any type of chest CT scan or lung scan in the past 11 months may not be eligible.
- People with a prior history of lung cancer, either recorded in their medical file or reported by themselves, may not be eligible.
- People who have been hospitalised within the last 60 days, either recorded in their medical file or reported by themselves, may not be eligible.
- People who have a diagnosis of dementia recorded in their medical file may not be eligible.
