"Who might be able to join this trial:

• The trial involves primary care practices (such as family medicine, geriatrics, or general internal medicine) that are part of one of the 4 health systems participating in the study.
• Participating practices must use their health system's main electronic medical record system, so that patient records can be searched appropriately.
• At least half of the doctors and advanced care providers at a practice must agree to take part.
• People aged 50 to 77 years old may be eligible at the patient level.
• People who have a scheduled in-person or telehealth visit with a primary care provider at a participating practice within the next 14 to 25 days may be eligible.
• People who have smoked the equivalent of at least 20 packs of cigarettes per year over their lifetime (based on self-reported smoking history) may be eligible.
• People who currently smoke, or who quit smoking within the past 15 years, may be eligible.
• People who are able to communicate in English or Spanish may be eligible.

Who may not be able to join:

• People who have had any type of chest CT scan or lung scan in the past 11 months may not be eligible.
• People with a prior history of lung cancer, either recorded in their medical file or reported by themselves, may not be eligible.
• People who have been hospitalised within the last 60 days, either recorded in their medical file or reported by themselves, may not be eligible.
• People who have a diagnosis of dementia recorded in their medical file may not be eligible.
