Who might be able to join this trial:

• People between 18 and 85 years of age who are scheduled for a full bladder removal surgery (cystectomy), performed either as open surgery or using a robotic approach.
• People whose surgery will include a specific type of urinary diversion — either an ileal conduit (where urine drains into an external bag) or an ileal neobladder (where a new bladder is created from part of the intestine).

• People who have taken more than three doses of opioid pain medicines (by mouth or injection) in the seven days before surgery.
• People whose cancer has spread to other parts of the body (metastatic cancer).
• People currently receiving treatment for another type of cancer at the same time.
• People who have received chemotherapy for bladder cancer within one month before the scheduled surgery (prior chemotherapy given before surgery to shrink a tumour is generally allowed — confirm with trial site).
• People who are pregnant, breastfeeding, or who are of childbearing potential and are not using an accepted form of birth control (confirm with trial site for the full list of accepted methods).
• People who have taken part in another clinical trial for a drug or medical device within 30 days of surgery, or who plan to join another such study during this trial.
• People who have significant abnormal blood or lab test results at the screening stage that would have led to the cancellation of surgery.
• People who are currently using illegal drugs or misusing alcohol.
• People with a history of past surgeries, illnesses, or behavioural or mental health conditions (such as depression or psychosis) that, in the opinion of the trial doctor, could affect the results or pose additional risk.
• People with severe dementia that affects their ability to carry out daily activities.
• People with severe liver disease, classified as Child-Pugh Class C (people with milder liver disease in Class A or B may still be considered).
• People with a history of heart failure or current heart failure.
• People with very advanced kidney disease, where the kidney filtration rate is below 15 (end-stage renal disease).
• People with a complete blockage of the digestive tract.
• People who have a feeding tube (such as a gastrostomy or jejunostomy tube) or who require tube feeding.
• People who do not speak English.
