Who might be able to join this trial:
- People who are over 18 years of age
- People whose patient or main carer can read and speak English
- People whose patient or substitute decision maker is able to give informed consent to participate
- People whose patient or main carer is mentally and physically able and willing to use a mobile app called VIEWER and take their own health measurements at home (such as weight and blood pressure)
- People with advanced kidney disease (stage 5 chronic kidney disease), meaning two separate test results showed very low kidney function, or people whose kidneys have a greater than 40% chance of failing completely within the next 2 years based on a standard medical calculation
- People who are already being seen and managed by a specialist team at a kidney disease clinic
Who may not be able to join:
- People who are unable to use the VIEWER app for self-monitoring, either on their own or with the help of a carer
