Who might be able to join this trial:
- You are 18 years old or older (if you are over 80, you must also be assessed as not severely frail, based on a standard frailty scoring tool)
- You have moderate or advanced chronic kidney disease, based on a specific kidney function measurement (eGFR between 15 and 44.9, confirmed by a blood test)
- You have high blood pressure that is being treated with at least one blood pressure-lowering medicine from a specific group of drugs (called ACE inhibitors or ARBs), at the highest dose you can tolerate, and your treatment has been stable for at least one month
- Your blood pressure is still not well controlled, shown by readings taken both at a clinic and at home or during a 24-hour blood pressure monitoring test
- You are covered by or entitled to social security
- You are willing and able to give written consent to take part
Who may not be able to join:
- You are currently subject to any form of legal guardianship or protection order
- You are pregnant or breastfeeding
- You are a woman who could become pregnant and are not using a highly effective form of contraception (such as hormonal contraception that stops ovulation, a coil/IUD, or a hormonal intrauterine device)
- You show signs of being significantly dehydrated or low on body fluids
- You experience dizziness or faintness when standing up due to low blood pressure
- Your blood sodium level is too low (below 130 mmol/L, confirmed by a blood test)
- Your blood potassium level is too low or too high (outside the range of 3.5–5.5 mmol/L, confirmed by a blood test)
- You have had a serious heart or stroke event in the last three months, such as a heart attack, a hospital stay for heart failure, or a stroke
- You are currently receiving chemotherapy for cancer
- You have had an organ transplant (such as a kidney, liver, or heart transplant)
- You are already taking two or more types of water tablet (diuretic) medicines at the same time
- You are currently taking a type of medicine called a mineralocorticoid receptor antagonist (such as spironolactone) (confirm with trial site)
- You have a specific inherited kidney condition called autosomal dominant polycystic kidney disease and are being treated with a medicine called Tolvaptan
- You have a medical reason why you cannot take certain water tablet (diuretic) medicines used in this trial (confirm with trial site)
- You have severe heart failure (confirm with trial site)
- You have advanced liver cirrhosis (confirm with trial site)
