Who might be able to join this trial:
- You have been diagnosed with progressive multiple sclerosis — either secondary progressive MS (SPMS) or primary progressive MS (PPMS) — based on recognised medical criteria.
- You are between 18 and 65 years old.
- Your level of disability falls within a specific range on a standard MS disability scale called the EDSS, scoring between 3 and 6.5 (confirm with trial site).
- You are able to complete a walking test called the Timed 25-Foot Walk (T25FW), which involves walking a short distance.
- There is documented evidence that your disability has been getting worse over the 24 months before the screening appointment.
- You may or may not be taking a stable MS medication, as long as the medication has not changed in the last three months.

Who may not be able to join:
- You have been diagnosed with relapsing MS (where symptoms come and go in flare-ups) rather than progressive MS.
- You have been diagnosed with cancer at the time of starting the trial.
- You have a past history of malignant melanoma (a type of skin cancer) or breast cancer.
- Your progressive MS is currently in a stable phase, meaning it is not actively getting worse.
- You are pregnant or currently breastfeeding.
- You have dementia or another condition affecting the brain or nervous system at the time of your first visit.
- You have a mental health or physical health condition that would prevent you from safely taking part in the study (confirm with trial site).
- You have taken high-dose vitamin B3 (niacin) supplements within the 30 days before joining the trial.
- You have a genetically confirmed condition affecting your mitochondria (the energy-producing parts of your cells) or your metabolism.
