Who might be able to join this trial:

• Children and young people between the ages of 4 and 21 years old.
• People who have been diagnosed with a specific type of brain tumour called Diffuse Midline Glioma, confirmed either by brain scan or by laboratory testing of tumour tissue, and whose disease has shown signs of getting worse.
• People whose general health and ability to carry out daily activities meets a minimum level, as measured by standard medical scoring tools (confirm with trial site for the specific score required).
• People whose blood counts are within acceptable ranges, meaning enough white blood cells and platelets, and normal blood clotting results.
• People whose kidneys are working well enough, shown by normal levels of certain minerals in the blood and adequate kidney function measurements.

• People who have waited the required amount of time after finishing previous cancer treatments before joining — the waiting period varies depending on the type of treatment previously received (confirm with trial site for specific timeframes).
• People who are on a stable or reducing dose of steroids and a stable dose of anti-seizure medication for at least one week before joining.
• People (or their parent or guardian, where applicable) who are able to give informed consent to participate.

Who may not be able to join:

• People who have previously received a chemotherapy drug called etoposide.
• People who, in the opinion of the lead doctor, would not be able to tolerate the study procedures or any anaesthesia required.
• People with seizures that are not well controlled.
• People who are pregnant or breastfeeding, or people of childbearing potential who have not agreed to use two effective methods of contraception during the study and for at least one month after the last study treatment.
• People who are currently taking part in another clinical trial or receiving other anti-cancer treatments at the same time.
• People whose heart rhythm test (ECG) shows an abnormal result above a certain threshold.
• People with signs of an active infection anywhere in the body.
• People with a known allergy to etoposide or to gadolinium-based contrast agents (used in certain types of brain scans).
• People who have implanted metal or electrical devices in their body.
• People with high blood pressure that cannot be controlled.
• People with a known bleeding disorder.
• People with a history of heart structure problems or irregular heart rhythms.
• People with a history of stroke without a clear cause, or signs of stroke in the area of the brain being targeted by treatment.
• People who were hospitalised in the past month due to COVID-19 and require blood-thinning medication as a result.
• People with blood clotting problems or who are currently taking blood-thinning medication.
• People showing signs that the brain may be at risk of dangerous pressure, or who have had bleeding within or around the tumour.
• People whose Diffuse Midline Glioma is located in the spinal cord rather than the brain.
• People taking another medication where there is a reasonable concern about harmful effects on the brain or nervous system.
