Who might be able to join this trial:
- People aged 18 or older
- People diagnosed with a type of cancer called squamous cell carcinoma (or a closely related type) affecting the mouth, throat, voice box, nasal area, or sinuses — or where the original source of the cancer is unknown
- People whose cancer has been assessed as moderately to severely advanced in size or spread, based on a standard cancer staging system (called AJCC 8th edition)
- People who have not previously had radiation therapy for this type of head and neck cancer in a way that would, in the treating doctor's judgement, affect the need for radiation after surgery
- People who are already planned to have surgery with the goal of curing the cancer at one of the participating hospitals
- People for whom the treating team expects to recommend radiation therapy after surgery — with or without chemotherapy — based on the likelihood of finding at least one of the following in the surgical specimen: advanced tumour stage (pT3 or pT4), cancer spread to lymph nodes, cancer growing along nerves, or cancer found in blood or lymph vessels
Who may not be able to join:
- People who are not able to speak or read English or Spanish
- People living with severe mental illness that would prevent participation in the trial
- People whose cancer is HPV-positive, located in the oropharynx or of unknown origin, and is at an earlier stage with only a single lymph node involved that is smaller than 3 cm on the same side as the tumour
- People who have another separate, untreated cancer at the same time that is expected to affect how long they are likely to live
