You have been part of the Epsilon care pathway since it opened in February 2022
You have been diagnosed with COVID-19 lung infection (confirmed by medical records) and were referred to the Epsilon service by a community/city doctor, AND you still have serious or ongoing symptoms that have lasted more than 4 weeks, or complex/disabling symptoms lasting more than 3 months
