Who might be able to join this trial:

• You must be willing to give your consent (or have a legal representative do so) to participate in the study.
• You must agree to allow researchers to use stored tumor tissue samples from a previous biopsy (exceptions may be possible if samples are unavailable — confirm with trial site).
• You must be 18 years of age or older.
• You must be well enough to carry out basic daily activities — not fully bedridden (confirm with trial site).
• You must have been diagnosed with a specific type of lung cancer (non-small cell lung cancer) that has grown or spread in 1 to 5 areas of the body while on or after receiving immunotherapy (with or without chemotherapy) for at least 3 months, with scans confirming this progression.
• All areas of cancer growth must be suitable for a precise type of radiation treatment called SBRT, and each active area must be at least 1 cm in size.
• The original (primary) lung tumor must have been stable for more than 3 months, OR if it is also progressing at the same time, it must be treated with the goal of controlling or curing it.
• At least one area being treated with radiation must receive a dose lower than a specific technical threshold due to nearby sensitive organs (confirm with trial site).
• If another type of local treatment (such as surgery or ablation) is more suitable for some areas, that may be allowed after discussion with the study doctor, but at least one area must still be treated with SBRT.
• If you have cancer that has spread to the brain, it must have been treated before joining the trial, and brain spots will not count as one of the 1–5 progression sites.
• You must be expected to live for at least 3 more months, in the opinion of your doctor.

Who may not be able to join:

• If the study doctor feels you are not suitable or unlikely to follow the study requirements, you may not be eligible.
• If a radiation specialist determines that SBRT is not a safe or suitable treatment for you, you would not be eligible.
• If you are unable to have a PET/CT scan, or your cancer does not show activity on a PET scan, you would not be eligible.
• If you are pregnant or breastfeeding, you would not be eligible; women who could become pregnant must have a negative pregnancy test within 72 hours before starting treatment.
• If the study doctor feels you may have difficulty following all study procedures for any reason (including practical or logistical issues), you may not be eligible.
