Who might be able to join this trial:

• You are willing to take part voluntarily, have signed a consent form, and are able to follow the study requirements
• You are 18 years of age or older, are reasonably physically active in daily life (as rated by your doctor on a standard scale), and are expected to live for more than 3 months
• You have been diagnosed through a biopsy with medullary thyroid cancer that cannot be removed by surgery and has either spread to other parts of the body or is locally advanced, and there is evidence it has been getting worse within the past 14 months
• You have at least one tumour that can be measured on a scan
• Your blood counts are at acceptable levels within 7 days before starting treatment — including enough red blood cells (without a recent transfusion), white blood cells, and platelets (confirm specific values with trial site)
• Your liver and kidney function blood tests are within acceptable ranges within 7 days before starting treatment (confirm specific values with trial site)
• If you are a woman who could become pregnant, you must agree to use effective contraception during the study and for 6 months after, have a negative pregnancy test within 7 days before joining, and must not be breastfeeding
• If you are a man, you must agree to use contraception during the study and for 6 months after it ends

Who may not be able to join:

• You have been diagnosed with another cancer in the past 3 years (some exceptions may apply, such as certain early-stage or fully cured cancers — confirm with trial site)
• You have had major surgery, an open biopsy, or a serious injury within 28 days before starting the study
• You have significant heart problems, such as moderate-to-severe heart disease, an irregular heartbeat, or heart failure (confirm specific thresholds with trial site)

• You need dialysis due to kidney failure
• You have another serious health condition that your doctor believes could put you at risk or affect the study results
• You have previously taken the study drug (anlotinib) or similar drugs used to treat cancer by blocking tumour blood vessel growth, such as vandetanib, cabozantinib, lenvatinib, sunitinib, or sorafenib
