Who might be able to join this trial:

• People aged 18 to 70 years old, of any gender.
• People who have been diagnosed through a tissue test with a specific type of nasopharyngeal cancer (NPC) that has come back or spread, and where the cancer cannot be treated with local treatments alone.
• People who have not yet received any systemic (whole-body) treatment for their recurrent or metastatic cancer — though previous radiation, chemotherapy given before or alongside radiation, or chemotherapy after radiation may be acceptable if it was completed at least 6 months before joining the trial.
• People who are generally well enough to carry out daily activities, rated as ECOG 0 or 1 (meaning fully active or restricted in strenuous activity but able to do light work) (confirm with trial site).
• People who have at least one tumour that can be measured using standard imaging criteria.
• People whose liver, kidneys, and other organs are functioning well enough, based on blood tests done during the screening period.
• People who are expected to live for more than 12 weeks.
• People who are able to understand and sign a consent form agreeing to take part.

Who may not be able to join:

• People considered at high risk of tissue breakdown in the nasopharynx area — for example, those who have had two rounds of radiation therapy and whose last radiation was within the past year (specific rules depend on the stage of cancer; confirm with trial site).
• People who currently have, or have previously had, another type of cancer — with some exceptions, such as certain treated skin cancers, treated cervical cancer, localised prostate cancer, or ductal carcinoma in situ that has been fully treated.
• People with active bleeding, open ulcers, a bowel perforation, or who have had major surgery within the past 30 days; also people whose tumour is located very close to major blood vessels, or who have had serious bleeding (more than 30 ml in the past 3 months), coughing up blood (more than 5 ml in the past 4 weeks), or a blood clot, stroke, or mini-stroke within the past 12 months.
• People with uncontrolled high blood pressure even with medication (above 140/90 mmHg), significant heart disease, certain irregular heart rhythms, or heart failure.
• People with known or suspected autoimmune diseases, including dementia or seizure conditions.
• People who have difficulty swallowing, chronic diarrhoea, or a bowel blockage that could affect how oral medications are absorbed.
• People who have taken high doses of steroid medications (glucocorticoids) within the 4 weeks before joining.
• People who need long-term use of immune-suppressing drugs or high-dose steroids for an ongoing condition.
• People who are HIV positive; people who have active hepatitis B (based on specific blood test results); or people with chronic hepatitis C confirmed by blood test.
• People who are pregnant, have a positive pregnancy test, or are currently breastfeeding.
